Request for a Change from T-Mobile Direct to Indirect with Dealer Authority Word Document If your customer wishes to move from T-Mobile direct to the indirect channel, and give you Dealer Authority on their account, this letter must be completed and sent to T-Mobile.	Indirect to Direct Request Word Document If your customer wishes to move to T-Mobile direct from the indirect channel, and remove your Dealer Authority from their account, this letter must be completed and sent to T-Mobile.

Request for a Change in Dealer Authority Word Document If your customer wishes to change which dealer has authority on their account, this letter must be completed and sent to T-Mobile.	Feedback Form Word Document If you or your customer have any issues on your customer's account, this form must be completed and sent to T-Mobile.
Request for an Addition to Dealer Authority Word Document If your customer wishes to add another dealer who has authority on their account, this letter must be completed and sent to T-Mobile.
